New Employees

Benefits are effective upon first day of employment, even though benefit selections may not be made until a few weeks later. Health/dental application forms will arrive by mail approximately 3 weeks from the date of employment. If you or a family member need to see a physician or dentist before you have completed and returned these forms, you will still be eligible for benefits, based upon the plan chosen. However, you will need to contact the OSU Benefits Office to discuss documentation they can provide to your physician verifying that you are covered by OSU benefits. The "Benefits Overview Booklet" is available at: http://hr.osu.edu/hrpubs/ben/overviewbook.pdf . Specific plan details for medical, vision and dental may be accessed from: http://hr.osu.edu/benefits/healthhome.htm

Health/dental cards should arrive approximately 2 weeks after completed application forms have been submitted to the OSU Benefits Office. If you do not receive your cards within this 2 week period, please call the number below for assistance:

Making changes to your benefit selections

Changes to your existing health, dental, or vision benefits plan may be made only during an Open Enrollment period, which occurs every October. Exceptions would be qualifying events such as marriage, divorce/dissolution, birth of a baby, dependent turns 23, obtain new coverage, or loss of coverage on a spouse's insurance plan. If you have a qualifying event, please complete and submit the Family Event/Address Change Form and send to:

IN ADDITION to the Family Event/Address Change Form, you will also need to complete: 1) the appropriate benefit form(s) based on your qualifying event by going to the following website: http://hr.osu.edu/events and 2) an updated W-4 (tax card)
